Dr. F. Ramzi Asfour: A Passion for Medicine, A Talent for Truth

Dreams and ambitions often evolve with time, shaped by life’s twists and turns. But for Dr. F. Ramzi Asfour, the path he set as a young boy never wavered.

“My close friend Jay and I went to pediatricians who were partners, Drs. Kent and Errante. Dr. Kent was also a troop leader, and we looked up to him,” he recalls. “We decided we would become doctors and take over their practice when they retired.”

He pauses with a smile. “Well, I never varied from that decision—but my friend is now a Jewish Rabbi.”

For Dr. Asfour, medicine wasn’t a default choice or fallback career. It was a calling, something both noble and deeply personal. From that early decision sprang a rich professional life that would take him around the globe—into boardrooms, academic classrooms, courtrooms, and eventually into the business world as the founder of Praxis Med Experts, a fast-growing team of physician expert witnesses committed to clarity, accuracy and integrity.

From Africa to the Golden State

Dr. Asfour earned his Bachelor of Science in genetics at UC Davis before attending New York Medical College, then completed his residency in internal medicine at California Pacific Medical Center and a fellowship in infectious diseases at UC San Diego.

In the years that followed, he amassed an impressive portfolio of credentials and experiences, including licensure in more than 10 states and faculty positions at top hospitals and medical schools. He has been a practicing clinician, a public health advocate, a mentor, and a problem solver.

But perhaps most notably, he has seen medicine through multiple lenses: global health, private practice, rural health, big city tertiary care, telehealth innovation, outbreak management, and now as an expert witness.

“There were three things I dreamed of when I was younger,” he says. “One was working for the World Health Organization, one was having a Swiss bank account, and the other was owning a BMW. I achieved all three, though only the WHO job lived up to expectations. The bank account I got primarily because I was living in Geneva at the time and only had $2,000 in it, and the BMW was 13 years old, had a dent in the passenger door, and smelled like gasoline inside when you drove it.”

While working for WHO, he traveled across many English-speaking parts of Africa—Ghana, Uganda, Botswana, Tanzania, Lesotho, Kenya, South Africa. “I’ve always felt a pull toward Africa,” Asfour shares. “I’ve never quite understood why Africa resonated with me so strongly. But one thing was clear, I wanted to be an infectious diseases doctor, in part so I could go where the need was greatest. Working in Ghana, during the HIV crisis, I witnessed the daily struggle for basic medical attention—it left an imprint on me.”

Back home in San Francisco, the struggles were different—homelessness, substance abuse—but it reaffirmed for him that the need for compassionate, competent care is universal.

He began working to address the “deserts” of care. “I developed my telemedicine infectious disease practice as a way to bridge the gap for rural communities in California,” he says.

Capsid Consulting, another of his companies, helps nursing homes improve infection prevention and care for residents—the impact of its work was most clearly seen during COVID. This all set the stage for the creation of Praxis Infectious Diseases—a clinical care company—and ultimately Praxis Medical Experts, which would become a game-changer for attorneys seeking physician expert witnesses.

How It All Began

Asfour stumbled into expert witness work almost by accident.

“The first time I was approached, it was just after returning from Geneva,” he says. “I was asked to collaborate on a case defending the VA in a malpractice lawsuit. I agreed—and the next thing I know, six banker’s boxes of paperwork arrive.”

He pauses. “I started sorting through everything—papers on my desk, on the floor, everywhere. After speaking with my wife, I decided I might take on more cases, but first, I was going to hire an assistant to help organize all the documentation. That was one of the best decisions I’ve ever made.”

The experience taught him two key things: first, how to methodically extract relevant facts from mountains of data; and second, that there was a real need for physicians who could not only interpret complex medical issues, but also communicate them clearly in legal contexts.

“It was like learning a new dialect,” he explains. “You have to be accurate, but also understandable. You have to know the medicine cold, but you also need to appreciate the legal process and the role you play within it.”

Praxis Med Experts: A New Model

Founded in 2022, Praxis Med Experts brings together a hand-picked team of board-certified physicians with real-world clinical experience who serve as consultants and expert witnesses in a wide range of cases—infectious diseases, cardiology, surgery, kidney disease, skilled nursing facility-related cases and more. Asfour’s goal is to cover all medical specialties.

Focused only on seeking and providing evidence of the truth, Praxis Med Experts offers services to both plaintiff and defense attorneys and emphasizes ethical transparency from the outset.

“My responsibility is to tell the truth,” Asfour says simply. “I always tell attorneys when they retain me: I’m going to review the records and give you my honest opinion. A good attorney will say, ‘That’s exactly what I want.’ And those are the ones I want to work with.”

Praxis is akin to a boutique firm. Cases are vetted. Experts are matched carefully. And there’s a heavy emphasis on quality, not volume.

“We’re physician-led,” he notes. “We take pride in the fact that our experts are clinicians first—people who still treat patients, teach residents, and stay current in their fields.”

This approach has paid off. In just a few years, Praxis Med Experts has grown through referrals and word-of-mouth into a trusted name among attorneys who deal with medically complex cases. Clients value the clarity and professionalism that Asfour and his team bring—especially in an era when COVID-19 and other infectious concerns have become frequent legal battlegrounds.

While some firms are still operating with processes that haven’t changed in decades, Praxis has built a model rooted in clinical clarity, careful vetting and transparency. For example, the firm offers a flat-fee initial opinion, helping attorneys quickly assess case viability without the usual ambiguity around costs.

“It’s still surprisingly rare in this space,” says Asfour. “But clear, upfront pricing just makes sense—for everyone involved.”

Lessons From the Courtroom

Though much of Dr. Asfour’s time is now spent running the business and mentoring new experts, he still takes on select cases himself. His portfolio includes criminal and civil cases, ranging from personal injury and product liability to wrongful death and medical malpractice.

One recent case stands out – a wrongful death suit related to workplace exposure to COVID-19.

“I was flying into Boston, just about to land, when I got a text from the attorney saying, ‘We’re about to settle,’” he recalls. “This was a workers’ compensation case in which the plaintiff claimed the decedent contracted COVID at work and died. But in reviewing the ambulance records, I noticed that when the wife made the call, she said it was her son who had COVID—not her husband.”

Asfour dug deeper. The son had, in fact, called in sick to work around that time. “I advised the attorney to verify those records. She did. And that new information changed the calculus. The case settled just before going to trial—and I got a few nice days in Boston.”

Stories like that aren’t rare in his line of work. Sometimes it’s a timeline discrepancy. Sometimes it’s a chart entry that contradicts a deposition. And sometimes, it’s just putting all the pieces together to show that an infection likely didn’t originate where someone claimed.

“That’s what good expert work is—it’s puzzle-solving,” Asfour says. “A good expert doesn’t have a side in the case; you’re trying to clarify the truth of the medical facts.”

Mentoring the Next Generation

One of the most gratifying aspects of Dr. Asfour’s current role is mentorship. Through his volunteer faculty appointment at UCSF, he teaches infectious diseases to internal medicine residents at Highland Hospital in Oakland. Through Praxis, he coaches physicians on how to become effective expert witnesses.

“It’s not enough to know the medicine,” he says. “You need to know how to prepare a report, how to explain medical concepts to a jury, how to withstand cross-examination. It’s a learned skill.”

He also talks about the emotional component, especially for doctors used to “fixing” things. “You’re not there to save a life. You’re there to tell the story of what happened, from a medical perspective. And sometimes the answer is uncomfortable or unclear. You have to be okay with that.”

While Praxis Medical Experts is still relatively young, its foundation is strong. The company’s success lies in the intersection of three key values: clinical expertise, clear communication and ethical rigor.

With rising demand for expert witnesses and increasing complexity in medical litigation, Praxis is well-positioned for continued growth. But Asfour insists the company’s future won’t be measured by size alone.

“I want Praxis to be known for integrity,” he says. “For doing good work. For being reliable. For helping the legal system make better-informed decisions.”

As our conversation winds down, it’s clear that Dr. F. Ramzi Asfour is a man comfortable in many roles: clinician, consultant, entrepreneur, mentor, family man. He still treats patients. He still teaches. He still reviews cases. And he still feels a sense of gratitude for the career he chose as a young Scout decades ago.

“My path has taken a few unexpected turns,” he says. “But I’m doing what I love. I get to help people—whether they’re patients, lawyers or young physicians finding their voice. That’s all I ever really wanted.”
